I ended up trying all the suggested solutions above and either one worked, or a combination worked, because my user can now see the Show Calendars element in the selection panel, and access the add facility.
I do not believe the ID recertification made any difference, and the additional icon column in the display had no effect either. The impression I get is that replacing the design with a bespoke template, then deleting the $Calendar view and (CalendarProfile) form, then re-assigning the required design template, resolved my particular issue.
